Preparing for your Visit
We understand that dental anxiety can be a common concern for many of our patients as they prepare for their appointments. Please know that our caring and dedicated Treatment Coordinators are by your side throughout your entire dental experience, and we hare here to provide you with personalized support.
To ensure your visit is as comfortable as possible, we have taken the extra step of creating a checklist of documents for you to bring along.
1) Dental Insurance
Don't forget to bring your dental insurance card!

Your insurance provider typically issues a card that includes coverage information:

Group Info
-
Coverage Dates

If it's under a guardian, at time of visit please additionally provide:

Guardian Name

Guardian Date of Birth

Guardian's Relationship

Guardian's ID Number

This can significantly expedite the processing of your insurance claims, making your visit more efficient and hassle-free.
2) Medications
Don't forget to bring your dental insurance card!

3) Referral Slips and X-Rays
If your dentist referred you to us, please bring the the referral slip/card. It may contain important treatment instructions.
This document often contains valuable treatment instructions. If you don't have it, you can request a digital copy from your dentist.

If you've had X-rays taken within the last six months, ask your previous dentist to send them to us. If we need additional X-rays, we can take them in-house in each treatment room.
4) Check-in Time
Please arrive 15 minutes before your appointment to complete registration or check-in. Bring your ID, insurance card, medication list, referral, and X-ray copies from your last visit.
